Evaluation of the effectiveness of bedaquiline and delamanid treatment among children and adolescents with multi-drug-resistant pulmonary tuberculosis

Introduction. Against the backdrop of multiple and widespread drug resistance Mycobacterium tuberculosis (MDR-TB), there has been a significant decline in effectiveness treatment (TB) patients Ukraine globally. Therefore, recent years, new antimycobacterial drugs, such as bedaquiline (Bdq), delamanid (Dlm) pretomanid, have introduced to improve efficacy adults, children adolescents. The purpose - study complex with (Bdq) under 18 years old extensively drug-resistant pulmonary TB (MDR/XDR-TB). Materials methods. To clinical chemotherapy Bdq Dlm, retrospective cohort analysis medical records was conducted. main group consisted 40 MDR/XDR-TB who received comprehensive therapy Dlm; control 27 without Dlm. Results. It found that during first three months treatment, decrease bacilli all treated Dlm these drugs (control), but group, significantly slower, p<0.05. According immune system parameters, after intensive phase completed, activity specific process 1.7 times more frequent than group. After completion course showed resorption infiltration, compaction foci, formation fibrosis lungs according results X-ray tomographic examination. However, 14.8% failure noted resumption bacterial release destruction lung tissue, had healing decay cavities. In majority (77.5%) resulted small residual changes, large changes were 2.3 common form dense Conclusions. We report the experiences of 5 patients taking bedaquiline with delamanid in combination: 1 patient was cured; 3 culture converted, with 2 continuing and 1 changing therapy; and 1 died from respiratory insufficiency. For 2 patients, QT-interval prolongation but no arrhythmias occurred. Use of this therapy is justified for patients with limited options.
